What can you do in the portal?
- Request annual well-visit appointments*
- Send and receive messages
- Login to Televisit appointments
- Request medication refills
- Access lab test results
- Access and print immunization records
- Pay balances and copays
- Keep track of scheduled appointments, you can even set an alert!!
*Please CALL the office if there is an emergent issue or a sick visit appointment is needed!!
How do you access the patient portal?
It is easy peasy! Using a desktop computer, laptop, or mobile device, follow these simple steps:
- Click on the Patient Portal button below
- Select “View Your Health Record”
- Login by selecting “Using Mobile Phone” NOTE: This option will send a verification code to your cell phone
- Enter the PATIENT’s first name, last name, and date of birth*
- Select which phone number to send the verification code to. NOTE: if this is the first time accessing the patient portal, you will be asked to set up a username, password, and security question. You will not need to use this in the future if you select Using Mobile Phone.
*Each patient/child has their own individual health record. You will need to log in to each of the charts individually.
Please note that once a patient has reached the age of 13, parents have limited access to the patient portal, and have restricted access after the age of 18. Proxy accounts can be set up for caregivers when necessary or when a patient has given written permission. If you are 18 or older and would like to grant proxy access please complete this form.
