Recommended Child and Adolescent Immunization Schedule for ages 18 years and younger

Newborn

2 Week

4 Week (must be on or after 4 weeks from 1st Hep B vaccine)

Gardasil – HPV vaccine (after 9th birthday – 2 doses 6 months apart)

11 Years

Age-appropriate developmental and or behavioral screens
Hearing & Vision
Hemoglobin (for all children entering 7th grade and girls with menses)
Urinalysis (for children entering 7th grade)


Vaccines:
Gardasil – HPV vaccine (2 doses 6 months apart)
Menquadfi (meningococcal vaccine)

12 Years – College Age
Height • Weight • Blood Pressure • Pulse
Age-appropriate developmental and or behavioral screens
Hearing & Vision
Hemoglobin (for girls with menses and children entering 10th grade)
Urine screen for chlamydia and gonorrhea (ages 16 and up)
Urinalysis (15-year-olds)
Lipid Screen (18-year-olds)

Vaccines:
Gardasil – HPV vaccine (if not given sooner)
Menquadfi booster (on or after 16th birthday)
Bexsero (Meningitis B Vaccine) 2 dose series minimum 6 months apart 

Tdap (at age 17, before college)
Catch up on vaccines, if needed

* Vaccines are safe and effective and are part of routine child and adolescent well-visits
